This is an annual UK wide initiative to help people to know more about diabetes. The Elms understand the importance of this and as always wanted to be involved.
So on Friday 14th June in honour of Diabetes Week we will have a diabetes dietitian from the Hertfordshire Integrated Diabetes Service (HIDS) running an interactive stand from 13:30 – 17:00 in our waiting room. This stand will have information about what diabetes is, an interactive carbohydrate game and information about how HIDS can help service users to manage their diabetes.
Hertfordshire Integrated Diabetes Service is a consultant led diabetes service aimed at providing high quality specialist diabetes care in the community. The service offers specialist diabetes multidisciplinary clinics in Herts Valley, structured diabetes education courses and aims to support GP practices in providing enhanced diabetes care to their patients in their practices.
The Diabetes Dietitians work alongside the Diabetes Specialist Nurses running specialist dietetic clinics and diabetes structured education courses.

New Practice Diabetes Service
We are delighted to introduce a new way of delivering our practice diabetes service in order to achieve the best possible care for our patients.
Our Diabetes Clinical Team consists of:
- Doctors: David Hemsi, Charlotte Allam and Vidya Kanthi
- Nurses: Paula McGinnigle and Judy Sutton
- Diabetes Specialist Nurse: Jo McIntyre
The new service design includes 3 types of diabetes clinics:
1) Diabetes Doctor Led Clinic
2) Diabetes Nurse Led Clinic
3) Joint Diabetes Doctor and Diabetes Specialist Nurse (DSN) Led Clinic
Annual Review Appointments
You will be invited for your annual diabetic review at ONE of the above mentioned clinics. Depending on your blood test results, we will choose the most appropriate clinic for you. Well-controlled diabetic patients will be seen by our practice diabetes nurses, whilst those who are less well controlled will be seen in either the doctor led or joint doctor and DSN led clinic. All appointments will be 30 minutes long.
Our Diabetes Clinical Team are highly-skilled and work very closely together in ensuring care is of the highest quality. We think this service will be well-received, and welcome any feedback from your experiences.
We will contact you when your annual review is due and advise you to book your first blood test (this is NOT a FASTING blood test).
Please collect a white topped urine sample pot from the practice reception. You will need to write your details on the bottle and bring this urine specimen with you, along with the enclosed form, to your blood test appointment, as no urine samples will be accepted without the form.

If you are on insulin or regularly monitoring blood glucose, please record your recent results. You can also use this document here:
Two weeks after your blood test please phone the surgery to book your annual review in the right clinic for you depending on your blood test result.
If for any reason you need to change the appointment please contact the surgery as soon as possible and rebook for another date. It is very difficult to rebook these appointments at short notice, so please give us at least a weeks notice so that we can offer the appointment to someone else.
